GP WrapAround

The High Fidelity WrapAround process developed by Wrap Canada will be used to support families to effectively attend to self-identified needs within a strengths-based model.

This program is a team-based, participant-led program. Participants are paired with a WrapAround Facilitator and WrapAround Clinician from our team, who can offer support on system navigation and counselling services. The participant will be provided an opportunity to work closely with Norwest service providers, including a Child & Family Services (CFS) liaison, Resource Centres, Indigenous elder(s), and other relevant supports. Participants decide on members they would like to include on their team to assist in accomplishing their goals. This could be family members, friends, service providers or people from any area of life.

Our team has the ability to navigate many different systems with the participant if required.

Examples include:

  • Income and Social Assistance
  • Child custody or care
  • Criminal justice system
  • Healthcare system
  • Housing

 

There are two streams for program entry:

  1. Child & Family Services (CFS)

Eligibility: This intensive program is available to individuals who are:

    • Aged 18 or older
    • Have current involvement, previous involvement or are at risk of involvement with Child & Family Services
    • Reside within: Gilbert Park, Blake Garden’s, Weston or Inkster area
    • Have the ability to meet weekly for check-ins or counselling
    • Are open to working as a team and choose team members from their own life

 

  1. Young Adult (YA)

Eligibility: This two-year program is available to individuals who are:

    • Aged 12-29
    • Have hopes, dreams or goals they would like to achieve
    • Reside within: Gilbert Park, Blake Garden’s, Weston or Inkster area
    • Have the ability to meet weekly for check-ins or counselling
    • Are open to working as a team and choose team members from their own life

9:00 am to 5:00 pm
